Summary

BASIC FUNCTION : Oversee nutritional care for patients seen in the pediatric gastrointestinal ambulatory care settings. Patient ages may range from neonates to young adult. Manage complex nutritional problems primarily related to a variety of conditions, both acute or chronic medical conditions. Recommend and calculate specialized nutrition regimens, including monitoring and implementing specialized nutrition support as needed. Orchestrate dietary interventions, nutrition education and comprehensive care management to optimize nutritional status, achieve growth and promote safe, positive health outcomes. Coordinate care on behalf of the patient to promote growth and self-care management. Identify, diagnose, and address pediatric malnutrition. Demonstrate flexibility in job role to meet patient, department, Registered Dietitian Nutritionist (RDN) team, and organizational needs.

Work full time, 40 hours/week, Monday - Friday.

Hybrid position with 1-2 days virtual work and 3-4 days/week onsite based on patient care needs.

ORGANIZATIONAL RELATIONSHIPS

  • Reporting structure:

  • Ambulatory Care RDN Supervisor for professional oversight

  • Ambulatory Care Nutrition Manager

  • Liaison relationship with interdisciplinary health care team for patient care populations of responsibility

  • Peer relationships with Registered Dietitian Nutritionists in work group

  • Preceptor to Dietitic Interns and students, as able

Responsibilities*

PRIMARY DUTIES AND RESPONSIBILITIES

Patient Care

  • Perform virtual visits in keeping with departmental and institutional guidelines when working remotely and in conjunction with on site, in person care at the Children's and Women's Ambulatory Care Unit (ACU) and occasionally at off site, nearby ACUs, as needed.

  • Conduct an ongoing comprehensive nutrition assessment for pediatric patients identified to be seen as per clinic standards.

  • Organize patient health and disease information to identify nutrition-related problems.

  • Diagnosis pediatric malnutrition after identifying the problem and clarifying the cause of the problem.

  • Develop and implement an individualized plan for nutrition intervention.

  • Analyze food diaries, meal recipes and specialized formula recipes for vital nutrients.

  • Ground interventions through review of quality research and/or use of medical nutrition therapy evidenced-based practice. Incorporate evidence-based standards when interpreting patient related data.

  • Monitor and evaluate need for continuation and discontinuation of specialized nutrition support and make appropriate recommendations accordingly.

  • Chart malnutrition diagnosis (if appropriate), individualized nutrition treatment plan, follow-up documentation of patient outcomes and revisions to plan in the electronic medical record. This communication is performed in a timely manner that is consistent with unit policy and procedures.

  • Obtain prior authorizations for nutritional supplements and support regimens as needed. Communicate with DME companies.

  • Recognize the varied needs of age-specific populations, as well as cultural, religious, and ethnic concerns.

  • Address patient's specific needs in the nutrition care plan and honor any advanced directives.

  • Participate, if required, in multi-disciplinary (CMDS) clinics

  • Measure effectiveness of nutrition interventions in reaching desired outcomes.

  • Reassess the nutrition care process and implement changes as indicated.

  • Provide short, frequent follow up care, most often through virtual visits, in keeping with best practices.

  • Communicate regularly with the medical team using both verbal and written communication

  • Communicate regularly and effectively with the inpatient registered dietitian nutritionists to provide continuity of care whenever patients transition between settings.

  • Abide by U-M Health policy for the proper use of patients' protected health information.

  • Abide by U- M Health policy for the proper use of infection control precautions in patient care areas.

Business Functions

  • Provide nutrition service to a wide variety of pediatric specialty clinics in the Children's and Women's Ambulatory Care Unit (CW ACU) for identified patient populations.

  • Implement charge capture by RDN for RDN services and facilitate scheduling patients as needed.

  • Provide quality service based on customer expectations and needs.

  • Incorporate Michigan Medicine core values of care, integrity, inclusion, teamwork, and innovation into all work efforts.

  • Exemplify and model equitable behavior that encourages belonging and respect in an inclusive, diverse work environment.

  • Maintain institutional and departmental productivity standards and practice effective time management.

  • Participate in developing nutrition-related policies, procedures, guidelines, protocols, and forms, particularly those associated with nutritional care in the CW ACU.

  • Assist in achieving compliance with state and federal regulatory and accrediting agencies.

  • Participate in interdisciplinary meetings, task forces, and projects as appropriate.

  • Contribute to quality improvement efforts including collection of outcome data to help improve practice in the CW ACU.

  • Participate in research studies and communicate findings through reports, abstracts, presentations, and publication as appropriate.

KNOWLEDGE AND SKILLS REQUIREMENTS

  • Able to work with a team to accomplish organizational goals.

  • Coordinate nutrition patient care services with the other Pediatric Specialty dietitians.

  • Cross cover other Pediatric Specialty dietitians during vacations, illnesses, etc.

  • Act as the point person to keep the rest of the team informed of the latest dietary trends in disease management

  • Communicate in a clear, concise and diplomatic manner both verbally and in writing.

  • Implement critical thinking to solve problems.

  • Use technology such as word processing, spreadsheets, e-mail, and telecommuting platforms.

  • Demonstrate flexibility in job role to meet patient, department, and organizational needs.

Required Qualifications*

MINIMUM CERTIFICATION

  • Registered Dietitian with active registration by the Commission on Dietetic Registration of the Academy of Nutrition and Dietetics. Requires primary source verification. Candidate may also be RDN eligible as long as RDN status is achieved within 6 months of hire.

PREFERRED CERTIFICATION

  • Board Certified Specialist in Pediatric Nutrition (CSP) OR Certified Nutrition Support Clinician (CNSC)

PREFERRED EDUCATION

  • Master's Degree in Nutrition related field.

  • Documented experience working with the medically complex pediatric specialty populations will be counted in lieu of an advanced degree.

PHYSICAL REQUIREMENTS

This position may require extensive daily walking/standing in the ambulatory care clinic environment to complete nutrition assessments.
