Fedcap Community Navigator in Hamilton, Ontario

Position Summary:

To liaise with community-based organizations and vendors that engage with individuals that are unemployed and may have barriers to employment such as living with a disabling medical and/or mental health condition, are justice involved, live in poverty, or are members of an identified inclusion group (Newcomer, Francophone, Indigenous, Racialized, Youth) or have other barriers to employment The Community Navigator, in partnership with the Service Delivery Team and outside organizations, will organize and manage referral pathways and tracking of service utilization. They will also be responsible for ensuring that individuals referred to Fedcap Canada are routed to an appropriate service provider in a warm and welcoming manner and follow up to ensure services have been received. This role is a key first point of contact for jobseekers and community partners and therefore requires exceptional communication and follow-through abilities.

Essential Responsibilities:

  • Respond to incoming referral requests by phone and email in a timely and friendly manner.

  • Establish rapport with jobseekers and facilitate warm hand offs to appropriate service providers in our network.

  • Follow up with jobseekers to ensure that services are received.

  • Track all contact and activity using office tools such as Monday.com, MS Excel, and/or the FedcapCARES case management system.

  • Provide outreach to community organizations by scheduling community-based partner meetings in-person or by phone and attending partner events to become familiar with available community resources suitable for unemployed and/or disabled individuals.

  • Develop and organize a resource repository to be shared with Service Providers to facilitate the referral of Service Provider participants to essential services.

  • In collaboration with the Community Partnership Manager, maintain the “Circle of Support” database, a community-based organization network which provides services and resources to participants, covering 26 domains of social determinants of health (health, safety and thriving).

  • Schedule and facilitate meetings between Service Providers and community partners as needed to optimize collaboration efforts.

  • Serve as conduit between Service Providers and community partners to support mutual participants’ success across both programs.

  • Prepare reports for various audiences on referral sources and success rates.

  • Provide administrative support to Community Partnership managers as it relates to community partnership organizations. Performs other duties as assigned. This job description is not designed to cover or contain a comprehensive listing of activities, duties, or responsibilities.

Education and Experience:

  • Diploma or Degree in Social Services, Public Administration, Public Health, Marketing, Human Resources, or related field.

  • Experience working with community-based organizations and/or government entities.

  • Experience working with individuals who have disabilities, live in poverty, and/or face other barriers to employment.

  • Knowledge of economic and workforce development and community health a plus.

Work Environment:

This job operates in a professional office environment. This role routinely uses standard office equipment such as computers, phones, photocopiers, filing cabinets and fax machines.

While performing the duties of this job, the employee is regularly required to talk or hear. The employee frequently is required to stand; walk; use hands to finger, handle or feel; and reach with hands and arms. Position Type/Expected Hours of Work:

This is a full-time position, and hours of work and days are typically Monday through Friday, 8:30 a.m. to 5 p.m. but will be determined by your supervisor. Occasional evening and weekend work may be required as job duties demand.

Travel:

Travel is primarily local during the business day, although some out-of-the-area and overnight travel may be expected.
