    About This Opportunity :

    Provides client centered services to individuals living with HIV. Assists clients with financial and community resources. Provides education on PrEP treatment, basic HIV and STI transmission, risk reduction strategies, nutrition, and medication adherence.

    Key Responsibilities:

  • Provide non-medical case management services to HIV+ clients through service planning and implementation, inter-agency collaboration and crisis management.

  • Support clients with budgeting and financial planning.

  • Provides education to clients regarding disease management and risk reductions.

  • Conduct a comprehensive assessment of client needs and make appropriate referrals.

  • Assure ongoing enrollment in 340B Pharmacy for eligible clients.

  • Maintain client records and charts with appropriate documentation.

  • Take active role with the program food pantry.

  • Other duties as assigned.

    What You Bring:

  • At least three years’ experience as a case manager, or public health experience, or associate degree in public health, Human Services, or related field preferred.

  • Bilingual (Spanish/ English) is highly preferred. Must be comfortable using a language service tool.

  • Experience working with electronic data systems

  • Excellent communication skills both verbal and written

  • Strong organizational skills and ability to work well independently and as a team

  • Must have a valid driver’s license and access to a reliable car

  • Acceptable CORI

  • Ability to function well in a high-paced and at times stressful environment.

  • Proficient with Microsoft Office Suite or related software.

    Physical Requirements:

  • Prolonged periods of sitting at a desk and working on a computer.

  • Must be able to lift up to 15 pounds at times.
