Title: Clinical Social Worker

What You’ll Do:

• Assesses and identify the client’s holistic health care needs/concerns as part of a family and community.

• Assesses social/emotional factors that are related to the patient’s illness, need for care, response to treatment, and adjustment to care.

• Develops a plan of care under physician orders with realistic/measurable outcomes in collaboration with the client/family and health care team providers according to Organization protocols.

• Implement patient care plan activities based on treatment goals towards independent living for the patient/family.

• Serve as resource for staff education & training, participate in case conferences as needed.

  What You’ll Bring:

• Master’s or Doctoral degree from a school of social work accredited by the council on social work education.

• Maine Board of Social Work Licensure: LCSW LCSW, LMSW-CC or LSW required.

• Current CPR card required.

• One or more years of experience as a master’s level social worker in a health care setting..

• A valid Drivers License and reliable car with insurance.
