The role you’ll contribute:

The Admissions Nurse is responsible for responding to and visiting with patients who have been referred for admission to AdventHealth Hospice Care. The Admissions Nurse informs the patients, caregivers, and family members of the hospice services available with the organization and assesses patient for admission to services. In addition, the Admissions Nurse is responsible for visiting current AdventHealth Hospice Care patients who have been admitted to the hospital and communicates patient status with the respective interdisciplinary team.

The value you’ll bring to the team:

  • Provides information and resources available to patients for admittance onto Hospice services.

  • Travels to resident site or hospital room of patient who has been referred to Hospice. Greets patient, caregiver and/or family member(s) and communicates the program and services available with Hospice.

  • Partners with the Account Representative staff member to meet the needs of the patients and hospital staff to gain new referrals.

  • Assesses the patient and family’s emotional condition as to readiness for hospice. Answers questions and responds to concerns to encourage admission onto the Hospice program if conditions are justified.

Qualifications

The expertise and experiences you’ll need to succeed:

  • Associate degree.

  • Minimum of three years of Registered Nurse experience in home health and/or acute care nursing experience; including 1-year experience in the hospital

  • An equivalent combination of education and experience may be substituted at the discretion of the Administrator

  • A clear/active Colorado Registered Nurse license.

  • Basic Life Support certification.

  • Maintain a current, valid Colorado driver's license in good standing with a safe driving record and a reliable vehicle.

  • Maintain valid personal automobile insurance.
