Scott County Public Health Nurse in Shakopee, Minnesota

Job Summary The eligible register developed from this posting may be used to fill future FT and PT vacancies within this classification. Performs responsible public health nursing duties through assessment, plan development, implementation and evaluation activities aimed to improve the level of health in Scott County. Plans, implements, and evaluates all grant responsibilities related to Child and Teen Checkups and Early Childhood Screening. Work is performed in an office, institution, community, home, school or community-based setting in the areas of health promotion and prevention. Essential Duties Performs independent health assessments of individuals, families and communities by evaluating their physical, mental, social, and environmental health status; provides nursing and community health intervention based on assessment of need and current community resources. Provides case management for clients by developing care plans, consulting with other health care providers such as physicians and social workers, referring clients to other health care resources, participating in team meetings, coordinating health care services, and instructing clients and families on methods of positive health maintenance; initiates and conducts interdisciplinary conferences. Provides leadership in projects related to the development, implementation, evaluating and maintenance of public health programs. Develops, implements, coordinates, and evaluates interventions in order to meet grant and program objectives. Consults, informs, and educates different audiences via public speaking, social marketing, mass media campaigns, and the development of educational materials. Represents the Department and the County both internally and externally in state, county, local communities, work groups, medical providers, and task forces in order to assure at-risk populations receive program services. Serves on internal and external work groups, task forces, and advisory committees. Recommends to senior leadership for advocacy initiatives for public health programs and resources. May assign tasks and gives daily work direction to Case Aides, Administrative Assistant, Retired Senior Worker, Community Health Workers, Community Health Specialists, and contracted Language worker in order to achieve a comprehensive team approach. Assists in evaluation of program outcomes including interpretations of reports to recommend on-going planning and establishment of intervention priorities. Proposes responses to identified needs using various methodologies. Recommends program changes to supervisor by researching health trends and community needs; by interpreting the results of data collection; by identifying resources; and by preparing reports and grant requests. Initiates contacts with potential grant sources and drafts recommended proposals for funding; manages grants by maintaining data related to activities and drafting periodic reports to grantors as required. Performs public health emergency response functions as trained and assigned within the department's emergency response plan. Participates in and takes direction to conduct disease investigation, and activate appropriate control and prevention plans as assigned. Assists to identify the gaps in the quality and accessibility of health care service for Scott County residents and make recommendations to management and policy makers for change. Works the days and hours necessary to perform all assigned responsibilities and tasks. Must be available (especially during regular business hours or shifts) to communicate with subordinates, supervisors, customers, vendors, and any other person or organization with whom interaction is required to accomplish work and employer goals. Performs other related duties as required or assigned. Minimum Qualifications Requires license to practice as a Registered Nurse (RN) issued by the State of Minnesota and CPR certification. Must obtain certification as a Public Health Nurse (PHN) in t e State of Minnesota by time of hire. Experience as an RN or PHN is preferred. NCAST, Carefacts, PHDoc, MMIS, DHS programmatic training, Catch 3, EVS, Follow Along, Immulink, NIMS IS700, IS100, IS200, and blood borne pathogen desirable. One must possess a valid driver's license and a reliable means of transportation for the performance of work responsibilities. Selection Process Selection for this position will be based on a minimum qualifications screening and rating of training and experience.
